I confirm that I am either the owner of the object being scanned or have obtained all necessary permissions to have it scanned.

I take full responsibility for downstream use of the files and acknowledge that OptimumARC Inc. does not claim ownership.

I confirm that, to the best of my knowledge, I am not experiencing symptoms of illness at the time of this appointment.

I understand that my visit involves interaction with shared equipment and public space.

Security Monitoring Consent *

I understand that security cameras are use at the OptimumARC Inc. facility for safety and liability purposes, and I consent to their use during my visit.

Photography & Recording Policy (3 boxes required) *

I acknowledge that photography or video recording of the scanning equipment, software interface, scanning room, or the scanning process is strictly prohibited.

I agree not to capture or share any images, videos, or recordings taken during my visit, unless prior written permission has been granted by OptimumARC Inc.

I may use my phone to scan QR codes provided by the system to download my files, but I agree not to take any photos or videos during the session.
